What do I need to bring to my appointment?
- A family member or support person
- Your completed patient information form
- Original referral, if you have a copy
- Medicare/DVA/Private health Insurance Cards
- All of your prescription and non-prescription medications, in their packaging
- A card for payment, as we are unable to accept cash
- Your glasses if you wear them
- Your hearing aid if you have one
- If your first language is not English, please let us know so that we can arrange an interpreter for you. (this is at no cost to you)
Your appointment
Please arrive 15 minutes prior to your scheduled appointment time to allow us to get you checked in before you see the doctor.
Whilst we schedule time for each appointment as appropriately as possible, on occasion, complex needs may require more time. You are welcome to contact the rooms an hour before your appointment to check how the doctor is running for time.
Please confirm your appointment by return text or contact the office by phone to reschedule.
Please provide 48 hours’ notice to if rescheduling, where possible. Be aware that no-shows and appointments cancelled within 48 hours, may attract a cancellation fee.
